New Student Enrollment

2025-2026 School Year


Documents the registrars will request:

- TWO proof of residency items from the EACS approved list dated within 30 days of enrollment

(Link: https://www.eacs.k12.in.us/departments/student_services/proof_of_residency  ) Note: Families who do not have these documents should discuss their situation with the Director of Student Services at 260-446-0289 right away to determine next steps.

- Parent's state-issued ID

- Original/certified copies of the students' birth certificate or other approved substitute

- All required immunization documentation

- Completed EACS Enrollment Form.

-Completed Enrollment Questionnaire.

- Social Security number, if applicable

- Copy of court orders regarding child custody, if applicable

- Copy of the current IEP for a student receiving special education services. (The school will still send an official request for records but having a copy available at enrollment can speed up the scheduling process.)

- Copy of current transcript/cumulative list of grades and courses taken for students in grades 10-12, if available. (The school will still send an official request for records but having a copy available at enrollment can speed up the scheduling process.)
